Job Description

MRD - Executive is responsible for managing medical records, patient files, documentation, data entry, record storage, retrieval, and confidentiality in the Medical Records department. The requires any graduate qualification, with medical records qualification preferred, and 2 to 4 years of experience in MRD, patient file management, documentation, record keeping, and healthcare data handling.


Role & Responsibilities

  • Maintain patient medical records in a systematic and secure manner.
  • Collect, verify, arrange, and file OPD and IPD patient documents.
  • Ensure discharge summaries, consent forms, case sheets, and clinical records are complete.
  • Manage record indexing, numbering, storage, tracking, and retrieval.
  • Maintain confidentiality and security of patient medical information.
  • Coordinate with doctors, nurses, billing, admission, and administration teams for records.
  • Track incomplete files and follow up for missing documents.
  • Support audit, insurance, legal, and administrative documentation requirements.
  • Maintain MRD registers, file movement logs, and daily work reports.
  • Scan, upload, digitize, or archive records as per hospital policy.
  • Ensure accurate data entry and proper classification of patient records.
  • Handle authorized medical record requests from internal departments.
  • Follow hospital documentation, retention, and confidentiality guidelines.
  • Support quality, compliance, and NABH-related documentation when required.
  • Maintain disciplined, accurate, and compliant medical record operations.
