MRD Executive

The MRD Executive will play a critical in maintaining, organizing, and retrieving patient medical records, ensuring data accuracy and compliance with legal and regulatory requirements. This requires strong attention to detail, knowledge of medical documentation, and the ability to handle sensitive information securely.

Roles and Responsibilities:

• Maintain and manage patient medical records in both physical and electronic formats.
• Ensure accurate and complete documentation of patient files including admission, treatment, and discharge details.
• Retrieve medical records promptly for doctors, audits, legal requirements, and insurance purposes.
• Verify and check records for completeness, accuracy, and proper documentation.
• Ensure proper filing, indexing, and storage of medical records for easy accessibility.
• Maintain confidentiality and security of patient information at all times.
• Coordinate with doctors, nurses, and departments for missing or incomplete records.
• Assist in medical record audits and ensure compliance with hospital policies and regulatory standards.
• Support coding andification of diseases as per standard coding systems (ICD, CPT if applicable).
• Handle medico-legal cases documentation and ensure proper record maintenance.
• Ensure timely submission and closure of inpatient files after discharge.
• Manage record retention, archiving, and disposal as per hospital guidelines.
• Assist insurance and billing teams by providing required documentation for claims.
• Maintain logs and registers for record movement and tracking.
• Support digitization and electronic medical record (EMR) system updates.
• Ensure compliance with NABH/JCI documentation standards.
• Prepare reports related to medical records and documentation status.
• Handle requests for medical records from patients or authorized parties.
• Ensure proper labeling, scanning, and uploading of documents into the system.
• Participate in quality improvement initiatives related to documentation processes.
• Coordinate with IT teams for smooth functioning of EMR systems.
• Maintain cleanliness and organization of MRD storage areas.
• Work in rotational shifts if required.
• Contribute to improving efficiency and accuracy of medical record management systems.
