Junior Executive - Medical Record

The Junior Executive – Medical Record will be responsible for maintaining, organizing, and updating patient medical records within the hospitals Medical Record Department. This involves accurate documentation, file management, report coordination, data entry, record retrieval, and compliance with hospital confidentiality standards. The candidate should have strong attention to detail, computer knowledge, and the ability to support smooth medical record operations while ensuring patient information is handled securely and accurately.

Roles & Responsibilities

• Maintain patient medical records, case files, discharge summaries, and clinical documents accurately.
• Organize physical and digital medical records as per hospital policies and departmental standards.
• Enter, update, and verify patient information in hospital record systems.
• Retrieve medical records for doctors, departments, audits, insurance, or administrative requirements.
• Ensure proper filing of investigation reports, consent forms, prescriptions, and treatment documents.
• Maintain confidentiality and security of patient health information at all times.
• Support discharge documentation by coordinating with doctors, nursing staff, and billing teams.
• Check medical files for completeness, missing documents, and documentation errors.
• Prepare medical record reports, file movement registers, and department trackers.
• Assist in coding, indexing, scanning, and archiving medical records when required.
• Coordinate with clinical, billing, insurance, quality, and administration departments for record-related work.
• Follow hospital documentation policies, legal requirements, and data protection practices.
