Assistant

The Assistant Medical Records Officer will support the hospitals medical records department by maintaining accurate, confidential, and well-organized patient documents. The candidate should be able to handle inpatient files, outpatient records, discharge summaries, consent forms, investigation reports, medico-legal documents, and record retrieval requests. This requires attention to detail, documentation discipline, confidentiality, computer knowledge, and coordination with doctors, nursing, billing, TPA, administration, and patient care departments.

Roles & Responsibilities

• Maintain patient medical records, hospital files, discharge documents, consent forms, and investigation records.
• Verify completeness of patient files, doctor notes, nursing records, discharge summaries, and required documents.
• Support medical record filing, scanning, indexing, data entry, retrieval, and archiving activities.
• Coordinate with doctors, nursing, billing, TPA, administration, and patient care teams for pending documents.
• Maintain confidentiality and security of patient information and hospital records.
• Prepare medical record reports, file trackers, missing document lists, and documentation updates.
• Support record retrieval for audits, patient requests, insurance claims, legal needs, and internal requirements.
• Ensure proper numbering, labeling, storage, and traceability of medical records.
• Identify incomplete files, missing signatures, wrong entries, or documentation gaps and follow up promptly.
• Use hospital software, Excel, scanners, printers, and office tools accurately.
• Follow hospital medical record policies, documentation standards, and confidentiality rules.
• Ensure organized, accurate, and audit-ready medical record management.
