The Medical Transcriptionist will prepare accurate healthcare documents, discharge summaries, medical records, typed clinical notes, and patient documentation at Sterling Hospitals, Gurukul, Ahmedabad. This requires Any Graduate qualification with Medical Transcription qualification preferred and 1–4 years of experience in medical transcription, medical records, typing, documentation, medical terminology, discharge summary preparation, and healthcare documentation.
Roles & Responsibilities
- Prepare accurate discharge summaries, clinical notes, patient reports, and healthcare documentation.
- Transcribe doctor dictations, handwritten notes, clinical instructions, and patient treatment details correctly.
- Use proper medical terminology, diagnosis details, procedure names, medication details, and follow-up instructions.
- Coordinate with doctors, nursing staff, medical records department, billing team, and discharge desk for required information.
- Review patient files, investigation reports, progress notes, prescriptions, operative notes, and consultant remarks.
- Ensure discharge summaries are complete, properly formatted, and ready within required turnaround time.
- Maintain typing accuracy, grammar quality, formatting consistency, and documentation clarity.
- Identify missing clinical information and coordinate with concerned departments for correction.
- Maintain confidentiality of patient medical records, treatment details, and hospital documentation.
- Use hospital software, MS Word, typing tools, and medical record systems efficiently.
- Maintain discharge summary logs, pending documentation trackers, and daily work reports.
- Follow hospital documentation standards, medical records policy, and patient data privacy guidelines.
- Support audit readiness through accurate and complete medical documentation.
- Escalate unclear notes, incomplete files, or urgent discharge requirements to seniors promptly.
- Deliver clear, structured, timely, and clinically reliable transcription support.
