Patient Care Coordinator

We are looking for a Patient Care Coordinator for the Patient Care department at Manipal Hospitals, Bengaluru. The candidate will coordinate patient services, home care support, appointments, communication, and patient experience activities. The requires strong communication skills, healthcare coordination ability, patient-focused behaviour, and knowledge of hospital administration processes.

Role & Responsibilities:

• Coordinate patient care services between patients, doctors, nurses, and support teams.
• Assist patients with appointments, service guidance, follow-ups, and hospital processes.
• Support home care coordination and ensure timely communication with patients.
• Handle patient queries and provide accurate information about services and procedures.
• Track patient requirements and coordinate with relevant hospital departments.
• Maintain patient records, coordination notes, and follow-up documentation.
• Ensure smooth patient movement through registration, consultation, billing, and discharge.
• Communicate politely with patients and attendants to improve service experience.
• Escalate patient concerns or service delays to seniors for quick resolution.
• Coordinate with nursing, pharmacy, diagnostics, and administrative teams when required.
• Support patient feedback collection and service improvement activities.
• Maintain confidentiality of patient information and hospital communication.
• Prepare daily updates or reports related to patient coordination activities.
• Ensure professional, empathetic, and patient-friendly communication at all times.
