Clinical Pharmacist

The Clinical Pharmacist will be responsible for medication review, prescription evaluation, drug safety monitoring, patient counselling, adverse drug reaction support, and clinical pharmacy coordination at Royal Care Hospitals, Coimbatore. This requires Pharm.D or B.Pharm with State Pharmacy Council registration and 1–2 years of experience in clinical pharmacy or hospital pharmacy services.

Roles & Responsibilities

• Review prescriptions for dose, drug interaction, contraindication, duplication, and safety.
• Support medication reconciliation and clinical pharmacy documentation.
• Counsel patients on medicine use, dosage timing, precautions, storage, and adherence.
• Coordinate with doctors, nurses, pharmacy, ICU, and patient care teams.
• Monitor adverse drug reactions and medication-related issues.
• Support antibiotic stewardship and rational drug use where applicable.
• Maintain clinical pharmacy records, intervention notes, and medication review reports.
• Verify high-alert medicines and support safe medication administration practices.
• Report medication errors, drug reactions, or safety concerns to seniors promptly.
• Assist in patient discharge medication counselling when required.
• Follow pharmacy regulations, hospital SOPs, and patient safety standards.
• Promote safe, effective, and quality-focused medication practices.
