Patient Care Coordinator

The Patient Care Coordinator will be responsible for guiding patients through hospital services, coordinating appointments, supporting admissions and discharges, and ensuring smooth communication between patients and departments. This requires empathy, communication skills, service orientation, and hospital workflow understanding. The candidate will help improve patient experience by resolving queries, reducing delays, coordinating care-related activities, and supporting respectful patient-centered service delivery.

Roles & Responsibilities

• Guide patients and attendants regarding hospital services, departments, procedures, and care processes.
• Coordinate appointments, consultations, investigations, admissions, discharges, and follow-up visits.
• Communicate with doctors, nurses, billing, pharmacy, diagnostics, and support teams for patient needs.
• Assist patients in completing required forms, documentation, and service-related formalities.
• Track patient movement, waiting time, pending reports, and service delays for timely coordination.
• Handle patient queries, concerns, and basic complaints with empathy and professionalism.
• Provide clear information about consultation flow, investigation process, billing steps, and discharge requirements.
• Support inpatient and outpatient coordination to ensure smooth patient experience.
• Maintain patient coordination records, follow-up notes, and service status updates.
• Escalate unresolved concerns or urgent patient issues to senior staff or management.
• Ensure patient comfort, privacy, dignity, and respectful communication throughout the care journey.
• Coordinate with housekeeping, security, and administration for patient support requirements.
• Maintain confidentiality of patient information and hospital communication.
